Internship Agreement

Completed by
Intern, with Supervisor and Professor

Description

The Intern is required to identify learning objectives for their internship.

The Supervisor must identify the internship activities to the Internship Office.

The Professor must then approve these learning objectives and internship activities to ensure they are achievable and appropriate. All information must appear on the Internship Agreement and is to be signed by all parties listed above.

The Internship Agreement lists all necessary internship details, including the student’s learning objectives, the organization’s internship activities, the internship dates and the contact information of the student and supervisor. This form must be signed by the student, the on-site supervisor and the assigned professor.

Note: This form must be signed by all parties and submitted to the Internship Office prior to the internship start date.

Follow-up of First Term

Completed by

Description

This is a brief telephone call within the first 6 weeks into the internship, between the Internship Coordinator, the Supervisor and the assigned Professor to discuss the initial progress and integration of the intern into the internship environment. The intern is briefed accordingly.

End of Term Follow-up

Description

This is a brief telephone call within the last 6 weeks of the internship, between the Internship Coordinator, the Supervisor and the assigned Professor to discuss the intern’s performance. The intern is briefed accordingly.

Internship Academic Report

Description

This is an academic report prepared by the Intern about the internship activities performed during the internship which reflects their initial learning objectives. The evaluation criteria for the academic report is determined by the assigned Professor; he or she evaluates the report and assigns a satisfactory (S) or a non-satisfactory (NS) grade to the intern.

Internship Progress Report

Description

This report is completed by the Intern and used to keep track of the progression of their  learning objectives.

The Progress Report is a tool to help the student evaluate the progress of their learning objectives. Students must complete and submit this report to the Internship Office at the mid-term and at the end of the internship.

Evaluation

Completed by
Supervisor/Intern

Description

An evaluation is completed by the Supervisor at mid-term and at the end of the internship.

The Internship Evaluation must be completed and signed by the on-site supervisor and submitted to the Internship Office at the mid-term and at the end of the internship.

Note: This evaluation is shared with the student and the assigned professor to help grade the student’s internship progress.
